Application Materials
The Executive MS-ISOM (SCM) Program admits students for spring semesters. Admissions decisions are made on a rolling basis; students may apply up to a year in advance of when they intend to enroll.
Deadline: All application materials must be received by December 1st of each year.
***Please note that the application process involves two offices and materials must be sent to both, unless otherwise indicated. Due to the volume of applications that are received, we do not provide an internal courier service, so materials should be sent in separate packages to the appropriate addresses to ensure their proper receipt.

Graduate Application
Submit OnlinePlease complete the graduate application and obtain your UF ID before sending the remaining application materials. The Application for Graduate Admission must be completed on-line as printed applications are no longer available. It is important to provide complete and accurate information. Application for Graduate AdmissionCertificate of Financial Responsibility
Send to the International CenterInternational applicants are required to document proof of financial support for the first year. Form I-20 or DS-2019, which is required for your VISA, cannot be issued until you have been admitted to UF and have completed and returned this form to the UF International Center (their contact information is on the form).
Certificate of Financial ResponsibilityGMAT/GRE Score
Send to the Office of Admissions and to the CSCMThe CSCM requires that applicants take all portions of the Graduate Admission Test (GMAT), or the Graduate Record Examination (GRE). Please visit www.mba.com or www.gre.org for test preparation materials. To register for one of the tests or to forward your official score to UF use the institution code of 5812. In addition, please hand-deliver or mail the department a copy of the official score report that you receive in the mail approximately two weeks after taking the test. Please do not fax the score report. Since it can take up to eight weeks for the Admissions Office to process your test score, it is important to allow for this delay by taking the exam well in advance of the application deadline.
Letters of Recommendation
Send to the CSCM onlyPlease use the form below to submit three letters of recommendation as part of your application to the Executive MS-ISOM (SCM) Program. If you waive your right of subsequent access to the recommendation letter, it must be sent in a signed and sealed envelope directly from the recommender to the CSCM (please do not email or fax). A recommender can be a professor or supervisor under whom you have studied, taught or worked, and who is able to comment on your qualifications for graduate study.
Recommendation Letter FormOfficial Transcripts
Send to the Office of Admissions and the CSCMArrange to have official transcripts sent from each postsecondary institution attended (not needed if only attended UF). Send one set of documents to the Office of Admission and one to the CSCM. Students applying from another state university in Florida should request from their Registrar's Office that the transcript be sent electronically.
International applicants must submit two official copies in the native language of each transcript, mark sheet and degree/diploma statement from each university or college attended, along with official English translations. Send one set of documents to the Office of Admission and one to the ISOM Department. If you have questions regarding international transcripts, please send an email.
Resume
Send to the CSCM onlyPlease submit a copy of your most current resume to the CSCM (this may be sent via an email attachment).
Statement of Purpose
Send to the CSCM onlyPlease submit to the CSCM, a typed one to two page statement that emphasizes the short and long term goals you hope to achieve by pursuing your Executive Master's in Information Systems and Operations Management (SCM) (this may be sent via an email attachment).
TOEFL
Send to the Office of Admissions and the ISOM DepartmentNon-US and Puerto Rican applicants must have a TOEFL (Test of English as a Foreign Language) score of at least 80 (Internet-based test), 213 (computer-format test) or 550 (paper-format test). To forward your official score to UF, use the institution code of 5812 and the department code of 82.
Applicants who are citizens of a TOEFL-exempt country or spent at least one year as a full-time student at a college or university in a TOEFL-exempt country prior to attending UF are exempt from the TOEFL. You can read a list of TOEFL-exempt countries on the UF Graduate School website. Please note: time spent in ESL (English for a Second Language) programs does not qualify for TOEFL exemption.
